We accept most insurance plans. It is your responsibility to verify with your insurance plan that our office participates in your network. Copays, deductibles, and coinsurance are due at the time of service. Our office makes every attempt to work with patients without insurance. We offer a 20% discount to uninsured patients paying at the time of service. Our office accepts cash, personal check, Visa, MasterCard and Discover. If you are not able to pay at the time of service, please call our billing office prior to your appointment at (513) 420-8676 to set up suitable arrangements.
We are happy to complete any forms and medical record requests needed for your children. We require 24 hours for forms to be completed. We require a signed authorization form to release your child’s medical records. Due to the heavy volume of requests, please allow us a maximum of thirty days to complete the medical record request.
